Health West, Inc. is a non-profit Community Health Center that provides primary care, behavioral health, pediatrics, and dental services to patients in our 19 locations throughout Idaho, Utah and Wyoming.

This provider shall be a fully accredited Family Nurse Practitioner licensed to practice medicine in the State of Utah. The provider works with the APP Director who evaluates progress and advises when appropriate. In the required duties of this position, reasonable exposure to blood borne pathogens is anticipated by association with patients, trauma, and performing lab work. The expected hours of work are Monday through Friday 9:00 a.m. to 5:00 p.m.

DUTIES:

1. Provides high quality, patient care. Care must be provided within the FTCA scope of services and defined by Health West (HW). Healthcare should include the use of HW clinical guidelines when applicable. Utilizes sound clinical judgement and problem-solving skills to address patient care problems.

2. Promotes a culture of safety and quality within worksites at HW including promoting team-based care, problem-solving, and maintaining standards of practice as described in the HW policies.

3. Fosters a spirit of collaboration, education and cooperation amongst the medical staff by being available for consultation with medical colleagues as requested, including other HW clinicians while maintaining flexibility to meet schedule and patient needs.

4. Promotes HW image as a health center striving to meet the health care needs of our community and advocating for high-quality health care for all, with particular focus on care for the underserved.

5. Maintains a work schedule as assigned by the Chief Medical Officer and participates in education sessions or medical staff meetings as required.

6. Acquires and maintains BLS/CPR certification (or ACLS).

7. Works closely with the Clinic Administrator and Team Leads of the clinic(s) to assist in establishing proper clinical activities and quality measurements. Clinical activities include, but are not limited to, Uniform Data System (UDS) requirements, as well as activities required by Health West for reporting to other payor entities.

8. Supports the Health West Director of Quality, the Director of Behavioral Health, the Chief Operations Officer, and other personnel as they implement workflows and other processes aimed at improving nursing, reception, behavioral health, prescription assistance, and outreach care.

9. Notes areas of medical deficiency within the clinic(s) and suggest ways to make improvements.

10. Is a positive team player, helping to maximize the good of the team.

11. Is amenable to clinic schedule changes so Health West can maximize patient care for the good of the patients and their respective communities.

12. Has reliable transportation, as the successful candidate will be asked to travel on occasion outside their home clinic location to provide primary care.

13. Reviews quality of care provided by other Health West medical personnel through periodic chart review and other reviews as required by the Chief Medical Officer.

14. Periodically presents medical topics in provider meetings for the edification of all Health West providers.

15. Periodically meets with the APP Director, Chief Medical Officer and the Chief Executive Officer to define clinical goals, personal goals, and review the progress of Health West operations.

16. Assumes additional duties as required by the Chief Medical Officer.

Performance Criteria:

1. Provides high-quality medical care to patients as evaluated by medical staff peers, the APP Director and the Chief Medical Officer.

2. Communicates effectively, particularly if supervising healthcare students.

3. Practices according to HW Standards of Practice including being punctual and reliable about showing up at scheduled times.

4. Maintains accurate and complete medical charts in a timely fashion including appropriate documentation of quality clinical measure.

5. Provides appropriate billing and coding information on chart audits as per CPT requirements.

6. Works well with clinic staff and other HW personnel.

7. Relates effectively with patients according to patient satisfaction surveys.

8. Completes diagnosis coding on all medical encounters.

9. Meets established productivity performance standards for clinicians.

10. Maintains confidentiality and follows HIPAA policies at all times.

11. Becomes an EMR superuser to maximize clinical efficiency and is able to provide quality data in a meaningful way, including UDS measures to the Health West network

12. Is supervised and evaluated by the Chief Medical Officer or his/her designee.

13. Other duties normally required for a Family Nurse Practitioner rendering primary care services.

Infection Control:

Applies handwashing and cleansing principles during daily work; demonstrates understanding of isolation precautions; complies with employee health policies; demonstrates understanding the process for identifying and handling infectious waste; maintains personal hygiene; demonstrates understanding of cross-contamination and complies with OSHA standards in the workplace.

OSHA Classification: Normal work routine involves potential for mucous membrane or skin contact with blood and/or body fluids; use of appropriate protective measures is required.

Qualifications

Minimum Qualifications:

Fully accredited Nurse Practitioner licensed with the Utah State Board of Nursing

Current BLS/CPR certification (may substitute ACLS)

Carries DEA and similar, applicable licensing.

Board Certified or Board Eligible (or similar designation)

Understands, or is willing to learn, the dynamics of underserved patient populations, including the homeless and migrant/seasonal farm workers.

Computer literate with credible typing, texting, and e-mail skills. Able to grasp Health West electronic workflows without difficulty (training provided).

Fluent in English.

Bonus Qualifications:

Experience working in a community health center

Experience working in primary care

Preferred bilingual in English and Spanish

Prior experience using Athena electronic medical record (EMR)

Prior experience using other EMRs
