Position Overview

We are seeking a Primary Care Physician with a strong interest in geriatrics, population health, and value-based care. This role goes beyond traditional medicine—it’s about managing a senior panel with a holistic, proactive approach that prioritizes prevention, cost-effective treatment, and quality-of-life outcomes. You will work closely with a multidisciplinary team to ensure our senior patients receive high-value, coordinated care.

Key Responsibilities

Deliver high-quality primary care to a panel of senior patients, with a focus on proactive chronic disease management and preventive care.

Monitor and manage population health metrics, including avoidable hospitalizations, emergency visits, medication adherence, and patient satisfaction.

Leverage data analytics tools and risk stratification models to identify high-risk patients and drive interventions that reduce medical costs.

Collaborate with care managers, pharmacists, behavioral health providers, and social workers to address social determinants of health and improve care outcomes.

Lead shared decision-making discussions with patients and families regarding advance care planning and cost-effective treatment options.

Participate in performance improvement initiatives related to quality metrics and total cost of care.

Maintain up-to-date documentation in the electronic health record (EHR) and contribute to accurate risk coding (HCC/RAF) to support appropriate reimbursement under value-based contracts.

Qualifications

MD or DO from an accredited medical school; board-certified in Internal Medicine or Family Medicine (Geriatric Medicine preferred).

Active medical license in Nevada

Experience or interest in value-based care, population health, or Medicare Advantage models.

Understanding of healthcare cost drivers, risk adjustment, and care utilization metrics.

Proven ability to work within interdisciplinary teams and manage a panel of complex senior patients.

Strong clinical judgment and communication skills, with a focus on patient-centered care.
