Position Details:
Full-time or Part-time options available with flexible schedules!
Locations: 1 Physician needed for Appleton and 1 Physician needed for Berlin/Oshkosh
The team consists of 5 Physicians and 8 APC’s
On-Call by phone only
Patient census: 8-12
Support staff includes RNs and schedulers who help to coordinate provider visits, communicate with facilities, and triage phone calls.
Will accept Internal Medicine or Family Medicine trained.
Senior Care Services
Visits completed at patient’s location (Nursing Home or Assisted Living)
Acute visits, compliance visits, & discharge visits for short and long-term residents
Advance Care Planning & Goals of Care discussions
Minor procedures at bedside
