Licensed Mental Health Counselor (LMHC)

The LMHC is responsible for evaluating, writing, monitoring, and coordinating behavior management plans with the clinical team in support of and consist with direct patient care.

Key Responsibilities

* Works directly in treatment relationship with patients and families, individually or in groups.
* Ensure service quality and participate in care coordination to ensure proper communication between clinical associates, patients, and referral sources.
* Participate in performance improvement activities
* Maintain current practice knowledge.
* Investigates case situations and presents information to Physicians and other members of the health team, on patient’s family and social background pertinent to diagnosis and treatment.
* Interprets psychiatric treatment to patient’s family and helps reduce the fear and other attitudes obstructing acceptance of psychiatric care and continuation of treatment.
* Refers patient or patient’s family to other community resources.
* Helps patient and family through individual or group conferences to understand, accept, and follow medical recommendations.
* Supports, consults and trains staff on substance abuse and mental health issues.
* Documents progress/data/plan of service in the Electronic Medical Record (EMR) utilizing objective, measurable methods.
* Consults with clinical team and participates in clinical team meetings.
* Writes psychosocial reports to be included as part of a patient’s Individual Plan of Service. Ensures report is included in Electronic Medical Record.
* Other duties, hours, and ACHS site placement may be assigned.
