Position Description
Our team of healthcare professionals positively impacts the health, well-being and quality of life for those we serve providing comprehensive, integrated, affordable healthcare and education to patients. We utilize a team-based model including embedded Behavioral Health Clinicians as well as Case Managers, Care Coordinators and Community Health Workers.
The Behavioral Health Case Manager assists the Behavioral Health team in providing coordinated behavioral health care to patients in an integrated primary care setting. Functions include: assessing clients’ needs, facilitating access to various resources, and advocating for clients to ensure they receive appropriate care and support. The case manager works collaboratively with healthcare providers, community resources, and patients to promote overall well-being and recovery.
QUALIFICATIONS:
Education/Training: Bachelor’s Degree or higher preferred.
Applicants without a Bachelor’s Degree may be considered if:
The applicant has an Associate’s Degree and at least 2 years experience working providing case management services; OR
The applicant has completed at least 50% of coursework required for a Bachelor’s Degree and agrees to complete the remaining coursework within a 3 year period.
Experience: Two (2) years of community health and/or case management experience preferred.
Must possess an understanding of grant related requirements and be responsible for reporting and clinical tracking.
Knowledge/Skills: Ability to work with technology such as computers, smart phones and/or tablets
Knowledge of medical terminology is preferred.
Bi-lingual skills helpful.
Responsibilities:
Monitors the electronic medical record (EMR) for the department and resolves, refers and/or escalates issues as appropriate.
Helps coordinate services between patients, providers and community agencies with the goal of reducing barriers to accessing needed services.
Records actions, interactions and outcomes directly in patient’s EMR. Documents activities, service plans, and results in an effective manner while strictly adhering to the policies and procedures in place.
Accesses and utilizes internal and external resources to assist with care coordination.
Answers phones, directs calls and responds to questions. Calls patients and other provider offices as required or directed.
Assists with screening new patients to determine appropriate level of care.
Processes providers’ lab orders.
Facilitates the warm handoff of patients between providers and staff.
Performs checks on clinical documentation to ensure all required information is included so billing can be completed in a timely way.
Serves as a resource to patients and staff on external/community behavioral health resources.
Assists patients and providers with initiating virtual care visits.
Supports and facilitates the smooth flow of patient care and related processes.
Assists with data collection and the development of quality and operational reports.
Supports internal review processes (peer review, performance evaluations) by organizing and distributing required forms and data.
Maintains confidentiality of patients and related documents at all times.
Provides exceptional internal and external customer service to patients and staff at all times.
Faxes and records successful transmission documents prepared by clinical staff.
Updates patient medical records by inputting data into the flowsheet.
Assumes responsibility for special projects as assigned by supervisor.
Reads and responds to email in no more than two business days, inclusive of internal newsletters and announcements.
Attends and participates in Amoskeag Health staff meetings and other meetings as directed; to include involvement in committees as requested. If absent, responsible for reading minutes of meetings and signing the documentation.
Attends mandatory safety-related and infection control in-services as assigned.
Maintains sensitivity to cultural diversity of population served.
Other responsibilities as assigned by manager.

Weekdays (8-5)
Compensation:
Compensation: The starting rate for this position is $18.00/hour. At the time of an offer, determination of your rate will reflect your skills and experience as it relates to the position.
