Reports to: Executive Director

Position Overview

The Nurse Practitioner plays a vital role in our care team, providing expert medical oversight for individuals and families. Basic job functions include diagnosing and treating common illnesses and injuries, developing personalized treatment plans, prescribing medications, and referring patients to specialists when needed.

Working collaboratively with a multidisciplinary team, the Nurse Practitioner will deliver patient-centered care with physician support available as needed. This is the perfect opportunity to make a meaningful difference in the health and well-being of the Sharon community!

Essential Services

* Contributes to treatment plan development and ensures accurate documentation of care plans, treatments, and patient responses.
* Updates and verifies patient information for personalized care and generates necessary orders and paperwork.
* Reviews charts, labs, and records promptly, and responds to phone messages in a swift manner.
* Maintains confidentiality, adheres to HIPAA policies, and assists patients with required forms.
* Completes notes, billing, and performs other duties accurately and on time.

* Makes prompt, independent decisions within expertise and scope of responsibility.
* Demonstrates strong problem-solving, critical thinking, and team leadership skills.
* Supports the Executive Director in achieving SCHC strategic goals.
* Upholds service excellence standards and fosters positive interactions with staff and providers.
* Adheres to departmental policies and remains adaptable to organizational changes, driving positive team dynamics.

Performance Improvement

* Participates in quality efforts and ensures compliance with HIPAA standards.
* Supports staff participation in SCHC quality and education efforts.
* Proactively seeks opportunities for improvement and demonstrates a willingness to learn.

Communication

* Provides timely, constructive feedback and actively listens using clarification techniques.
* Utilizes respectful, ethical communication to resolve problems and promotes peer collaboration.
* Facilitates upward communication and fosters a calm demeanor in high-stress situations.

Accountability

* Develops job descriptions, evaluations, and supports policy and procedure creation.
* Assists the Executive Director with staff management, including performance discussions, hiring, and terminations.
* Maintains punctuality, meets deadlines, and completes required training.
* Follows guidelines for record release and consistently applies administrative policies.
* Takes responsibility for personal behavior and performance.

Customer Service

* Demonstrates a service-oriented approach by conveying courtesy, respect, enthusiasm and a positive attitude in a work situation.
* Promotes good public relations between area agencies, hospitals, medical offices and insurance companies.
* Works cooperatively with the office staff, providers and management.

Security & Compliance

* Maintains the security and confidentiality of all patient records and SCHC information as written in our policies and procedures.
* Maintains the overall security of the facility and ensuring the facility is secure at the end of the day.
* Works closely with maintenance to provide a safe environment for employees and patients.

Minimum Qualifications

* A Bachelor of Science degree, followed by the completion of a Nurse Practitioner program.
* Required to have an active license in the state where they practice, and responsible for completing any state requirements to keep an active license.
* Required to maintain an up-to-date DEA license.

Technical Skills

* Able to operate a computer, fax machine, printers, scanners, webcam and other office equipment.
* Electronic Medical Records Experience, Centricity Experience preferred.
* Must have excellent oral and written skills and intermediate computer skills.

Other

* COVID vaccination required upon hire.
* Current CPR certification.
* Must maintain a valid driver’s license or dependable transportation.
