Position Description

POSITION SUMMARY:

Provides mental health case management to patients served by Warm Valley Health Care facilities, in particular: linkage, monitoring/follow-up, referral-assistance, advocacy, and crisis management

DUTIES AND RESPONSIBILITIES

Provides case management services largely consisting of:

—Linkage: Working with patients and/or service providers to secure access to needed services. Example activities include: communication with agencies/collaterals to arrange for appointments or services following the initial referral process, and preparing patients for these appointments; and making contact with hospitalized patients, hospital/institution staff, and/or other collaterals toward facilitating the patient’s reintegration into the community.
—Monitoring/Follow-up: Example activities include: contacting the patient and/or others to ensure that a patient is following a prescribed service plan; and monitoring the progress and impact of that plan.
—Referral: Example activities include: Arranging initial appointments for patients with service providers; informing patients of services available, addresses, and telephone numbers of agencies providing services.
—Advocacy: Engaging in advocacy on behalf of specific patients for the purpose of accessing needed services. Activities may include: making and receiving telephone calls; and completion of forms, applications, and reports which assist the patient in accessing needed services.
—Crisis Intervention: Providing intervention and stabilization of a patient in situations requiring immediate attention/resolution. The case manager may provide the initial intervention in a crisis situation, and would assist the patient in gaining access to other needed crisis services.

Completes all documentation at the time of an encounter, and no later than 24 business hours of the encounter—thus to maintain quality and productivity standards and up-to-date clinical records. This includes completion of progress notes and other relevant documentation, including (but not limited to) entry of relevant documentation into the electronic health record system.

Meets frequently with fellow Department/clinic treatment team members toward identification of patient needs and planning/coordination of services.

Attends in-service training and staff development programs to enhance case management services.

Performs other duties as assigned in order to maintain/enhance operation of the Department.

Adheres to, and abides by, rules of the Wyoming Professional Licensing Board and applicable Codes of Ethics for the profession.

Education & Experience Requirements:

Must have a bachelor’s degree or higher.

Must possess certification, or be able to obtain certification, through the state licensing board as a Certified Case Manager (CSW or CMHW). While the CSW/CSW-track applicant is allotted reasonable post-hire time to fully obtain the CSW certification, the CMHW applicant is required to fully possess the CMHW certification prior to hire.

Must be able to (a) understand the impact of trauma and responses to trauma, and (b) demonstrate sensitivity to, and acceptance of, individual experiences.

Must maintain and practice appropriate professional and personal boundaries.

Must maintain confidentiality in all interactions related to WVHC, for example (but not limited to) information related to patient status, patient records (and their contents), or other patient-related information. This requirement includes, but is not limited to, adherence to relevant HIPAA guidelines.

Must be flexible with scheduling, in order to meet the needs of the organization.

Must have the ability to work in a variety of settings with culturally-diverse patients, families, and communities, to include the ability to be culturally sensitive and culturally appropriate.

Must possess a valid driver’s license, and must be eligible/capable/willing to transport patients in a clinic-owned vehicle.
