Position Description
Fully competent in the primary role, including clinical documentation, prioritizing and responding to the clinical needs of the patients, and making value-added resource decisions for patient care.
Communicates and collaborates effectively with all members of the hospital team to address patient care issues and problem resolution and improve the delivery of multidisciplinary care.
Maintains and ensures department compliance with all established department policies/procedures, objectives, quality assurance program, compliance program, safety, environmental and infection control standards.
Ensures delivery of quality physical therapy services for inpatients and outpatients by completing initial evaluations in a timely fashion and developing quality physical therapy treatment plans.
Evaluate and develop functional maintenance programs for extended-care patients.
Prepares patient care documentation as required by professional and departmental standards. Documents records in accordance with accepted practices and all requirements – hospital, state & federal.
Provides individualized patient and family education and evaluates its effectiveness.
Coordinates with all members of the hospital team in order to address patient care, resolve problems, and improve the delivery of multidisciplinary care.
Uses verbal and written communication methods effectively.
Provides input into the refinement of physical therapy care plans.
Adheres reliably to all accepted patient safety standards
Assists in the maintenance of departmental facilities, equipment, supplies, and materials in a condition to promote efficiency, health, comfort, and safety of patients and staff.
Teaching and encouraging active involvement of patients and families in their care.
Follows all security, confidentiality, and privacy policies.
Active participant in quality improvement processes.
Enhances professional growth and development through participation in educational programs, current literature, in-service meetings and workshops.
Projects a positive image of the hospital through dealings with the public, patients, vendors, and fellow employees by acting in a professional manner at all times.
Observes hospital’s compliance requirements and does not create an issue of fraud and abuse for the hospital.
Assists in creating a program or programs that will benefit our Long-Term Care & Eagle Crest (assisted living center) patients and community.
Promotes positive relationships with physicians, employees, patients and community. Participates in community events and promotional activities as needed.
Participates in hospital staff committee meetings as directed.
Complies with operational policies and procedures for the physical therapy department. Complies with all established hospital policies and procedures.
Performs all other duties as assigned.

Education & Experience Requirements:
Bachelor’s Degree
Colorado Physical Therapist License
Current CPR certification
Work Schedule:
Weekdays (8-5)
Compensation:
