Selections made during enrollment may not be changed until the next open enrollment period unless
you have a qualifying event and notify the PVHC Benefits Coordinator within 31 days of that event.
Enrollment/Declination form must be completed within 31 days of hire
Effective the first of the month following date of hire
Premiums are deducted “pre-tax”
Premium deductions are bi-weekly (26 pay periods per year); full-time employees pays one-third
of total premium; part-time and prn employees pays two-thirds of total premium
Two plans to choose from: Advanced Plan and Basic Plan
Advanced Plan Benefits:
90/10 Plan for all allowable procedures (expenses for services performed at facilities other than PVHC
will be subject to reduced benefit levels)
Deductibles: $1,000.00 per individual/$2,000.00 per family (services performed at facilities
other than PVHC will be subject to higher deductibles)
Maximum out-of-pocket for covered expenses including deductible: $2,000.00 per individual;
$4,000.00 per family, (services performed at facilities other than PVHC will be subject to
higher maximum out-of-pocket limits)
Receive a 5% discount on hospital and clinic services performed at PVHC
Routine Care/Wellness benefits: 100% for adults and children; not subject to deductible
Prescription Drugs: generics from PVHC Pharmacy are paid at 100%; not subject to deductible
DENTAL INSURANCE (Benefits based on calendar year) – (Allegiance)
Effective the first of the month following date of hire
Twice yearly check-ups (preventive care) paid at 100% of UCR
Deductibles: $50.00 per person, $100.00 per family
Basic restorative: 75%; Major restorative: 50%
($1,500 maximum per calendar year for participants age 19 & over)
Implants: 50% ($2,500 per calendar year for participants age 19 & over)
Orthodontia: 50%; deductible waived (covers dependents through age 18)
($2,000 lifetime maximum)
VISION PLAN (Benefits based on calendar year) – (Allegiance)
Effective the first of the month following date of hire
No deductible
Benefits-(age 19 & over): Vision exam per calendar year paid at 100% of usual and customary
Lenses/Frames/Contact lenses: 100% up to $300.00 per calendar year
Benefits-(through age 18): Vision exam per calendar year paid at 100%
Lenses/Frames: 100% for one pair of glasses per calendar year
Contact lenses (in lieu of glasses): 100% one year supply
LIFE and ACCIDENTAL DEATH & DISMEMBERMENT INSURANCE – (UNUM)
Effective first of the month following date of hire
One times base salary ($300,000 max); premiums paid by PVHC
Employees may purchase additional term life and/or AD&D coverage
Spouse and child term life and/or AD&D coverage available for minimal cost
LONG TERM DISABILITY (LTD) – (UNUM)
Effective first of the month following date of hire
180 day elimination period from date of disability
Insures 50% of basic monthly earnings up to $7,500.00; cost paid by PVHC
RETIREMENT PLAN (Pension Plan) – (OneAmerica)
Available to all qualifying employees; must be eighteen (18) to be eligible
Effective first of the month following date of hire, part-time & full-time employees are eligible for employer contribution (based on years of employment): at onset 3%; at 10 years 3.5%; at 20 years 4%
Employees elect how funds are invested (Roth option available)
Employees may contribute up to maximum allowed by IRS rules
Withdrawals are governed by IRS rules
Vesting (based on years of employment): after 1 year 25%; after 2 years 50%; after 3 years 100%
529 TUITION SAVINGS PLAN
Tax free when used to pay qualified higher education expenses including tuition, fees,
room and board costs, books, supplies, computers and other equipment

HOLIDAY PAY
Effective at hire, there are 9 paid holidays, which includes 2 floating holidays (see HR policy 03.05)
PAID TIME OFF (PTO)
Effective at hire, available balance may be used with supervisor’s approval
May be used for: vacations, personal, first 2 shifts of sick time (see HR policy 04.03)
Accrues based on number of hours worked
0-4 years; may accrue up to 20 days per year (.0770 x hours worked); max. accumulation 200 hours
4-8 years; may accrue up to 25 days per year (.0962 x hours worked); max. accumulation 240 hours
8-12 years; may accrue up to 30 days per year (.1154 x hours worked); max. accumulation 280 hours
12+ years; may accrue up to 35 days per year (.1346 x hours worked); max. accumulation 320 hours
Carries over from year to year up to maximum accumulation
EXTENDED ILLNESS BANK (EIB)
Effective at hire, available balance may be used with supervisor’s approval
May be used after missing 2 scheduled shifts due to illness (employee/child/parent – see HR policy 04.31)
May use immediately if hospitalized, have oral surgery or out-patient surgery (employee only)
May accrue up to 7 days per year (.0269 x hours worked); max. accumulation 520 hours
Carries over from year to year up to maximum accumulation
FAMILY/MEDICAL LEAVE (FMLA)
Eligible after completing 12 months of employment
Must have worked at least 1,250 hours in the previous 12 months
Provides leave to employees with certain family and medical reasons
Upon approval and 30 day advance notice (absent an emergency),
up to 12 weeks of leave (use of PTO and EIB during this time off)
Continuation of benefits
Same or equivalent job upon return
BEREAVEMENT
Part-time & full-time employees may take 5 days up to a maximum of 40 hours paid time off
for death of immediate family member (parent, spouse, child, or step-child)
Part-time & full-time employees may take 3 days up to a maximum of 24 hours paid time off
for death of extended family members (as defined in HR Policy 04.04)
JURY DUTY
Facility will pay the difference between jury duty pay and the employee’s regular rate of pay
during regularly scheduled work hours
EDUCATION
Hospital based workshops and in-services (all shifts)
After 1 year of employment, Educational Cost Reimbursement Program
of up to 50% with a maximum of $5,000.00 per year on pre-approved courses
