Position Description

We are seeking a compassionate and responsible Wellness (Patient) Navigator, to coordinate care between patients and service providers within our facility, as well as in the community to assist in improving functional problems associated with a patient’s serious mental or physical health condition and to promote recovery.

Duties :

* Accurately record and track records in appropriate databases.

* Familiarize yourself with community services; supply and encourage patients to utilize information of community activities, resources, and services.

* Provide outreach by telephone and travel within the community to provide assistance.

* Arrange medical, social, and educational services to with providers, including care coordination functions.

* Develop and maintain relationships with other agencies who provide services to patients receiving case management.

* Be familiar with eligibility criteria, application processes, and appeal procedures for needed services.

Qualifications :

* Bachelor’s degree or equivalent experience required.

* Needs to be able to relate well to others, be organized, and possess clear written/verbal communication.

* Have excellent judgement and conflict resolution skills.

* Must use personal car for travel in community, valid driver’s license, acceptable driving record.

Work Schedule : Generally, Monday-Friday, full-time.

Education & Experience Requirements:

* Bachelor’s degree or equivalent experience required.
