Position Description
The Nurse Practitioner practices in collaboration with physicians and other healthcare staff to provide care management and care coordination. Provides direct patient care for men, women and/or children 12 years and over. Demonstrates ability to identify and document the needs of complex patients, identify, diagnose, and document chronic disease and perform comprehensive exams. Establishes a practice of using UDS measures for quality health care improvement. Works with GMH established nurse practitioner workflow schedule.

Requirements:
Qualified candidates must be Board Certified Family Nurse Practitioners with a valid/active Georgia Nurse Practitioner license.
Bilingual, English and Spanish, fluency highly preferred.
