Nurse Practitioner

Georgia Mountains Health (GMH) currently has an opening for a full time, experienced Family Nurse Practitioner with a minimum of two years clinical experience preferred for our Chatsworth, GA medical office. Georgia Mountains Health is a federally qualified health care center (FQHC) not-for-profit organization, committed to providing compassionate and quality to care to families in North Georgia, regardless of ability to pay.

The Nurse Practitioner practices in collaboration with physicians and other healthcare staff to provide care management and care coordination. Provides direct patient care for men, women and/or children 12 years and over. Demonstrates ability to identify and document the needs of complex patients, identify, diagnose, and document chronic disease and perform comprehensive exams. Establishes a practice of using UDS measures for quality health care improvement. Works with GMH established nurse practitioner workflow schedule.

Requirements:

Qualified candidates must be Board Certified Family Nurse Practitioners with a valid/active Georgia Nurse Practitioner license.

Bilingual, English and Spanish, fluency highly preferred.
