_About The Hospital_
Located just north of Guam in the Western Pacific Ocean, Saipan is the main island in an island chain that makes up the Commonwealth of the Northern Mariana Islands (CNMI), a commonwealth of the United States. The medical center is an 86-bed hospital and outpatient facility, located in the city of Garapan and serves a population of about 50,000 people.
The facility offers a broad range of services and specialties including General Surgery, Orthopedic Surgery, Obstetrics and Gynecology, Head and Neck Surgery, Ophthalmology, Dentistry, Podiatry, Oncology, Inpatient and Outpatient Internal Medicine, Pediatrics, Cardiology, Interventional Radiology, Emergency Medicine, and Nephrology.

_About The Practice_
At CHCC you will be part of a collaborative medical community providing services to the local population as well as tourists and other visitors. Transferring patients off the island for services is difficult and often not possible. As a result, General Surgery here is full spectrum and includes basic bread and butter services as well as endoscopy, trauma and critical care, some pediatric surgery, urology, some vascular surgery, and occasional neurosurgery. Don’t be intimidated by this! We are collaborative and work together as a team to provide care for our patients and to assist one another in being successful.

JOB DESCRIPTION:
We are a remote island, and the only hospital and ICU. Our patients have no other facility in general to divert to, no medivac service to a larger hospital. We are it. Our acuity runs from moderate to severe, most patients have multiple medical issues compounded by financial and social restraints.
Providers will be required to care for the total spectrum of adult medical illness (excluding OB/GYN), and will have available the assistance of UpToDate and experience of local team members. The EHR is CareVue. Except as noted below, subspecialty referral is NOT available and the successful provider will need to develop comfort in providing care in a resource limited environment. Nevertheless, the expected standard of care is that of a specialty clinic anywhere else in the US.
The local specialty backup available includes:
* Full-time: Internal Medicine; general surgery; orthopedic surgery; ENT; nephrology; pediatrics; psychiatry; podiatry
* Part-time: GI 2-wks/mo; interventional radiology 1-day/mo; locums orthopedics; on-call anesthesiologist
QUALIFICATIONS:
* Accepting permanent, one year or greater commitment
* Board certification required, or board eligible within 5 years of residency
* Must be IM or FP trained (U.S. or Canadian residency required)
* Must be Medicare reimbursement eligible
* ACLS/BLS required.
* Adult outpatient experience preferred but not required
* CNMI license required (application process supported by HR staff)
Licensing and credentialing takes 6-8 weeks (Process: Present CV and, if cleared, candidate will be notified by email with employment application attached. Three recent references are also required.)
SCHEDULE:
8 hrs/day, 5 days a week.
* If scheduling allows, the clinic may expand its hours to limited evenings and weekends.
* In such case, the provider schedule will be adjusted to provide 80 hrs every two weeks of duty time.
* Clinic visits are scheduled as half hour or one hour visits to provide time to address multiple issues in depth. NO quotas or production targets… Take the time you need to provide great care and research current practice guidelines. The buck stops here; we have to do it right.

As a result, earnings are taxed at the maximum CNMI (“state”) tax rate of 9% and there is a 90% rebate on U.S. federal income tax earned within the CNMI.
* If in the CNMI less than 183 days, the IRS taxes the CNMI income the same as U.S. mainland earnings for federal income tax purposes. For state taxes, the money earned in each state is paid at that state’s tax rate. Money earned in the CNMI has a maximum “state” tax of 9%.
* No sales tax.

Compensation:
$218,000 Per Annum (Salary is equivalent to that of a $330K salary in San Francisco and New York)
Benefits:
* $10,000 one-time signing bonus for initial contracts of at least 2 years.
* International flight from the point of recruitment for the employee. Return flight to original embarkation is included and will be arranged upon completion of contract.
* Malpractice coverage is provided under the CNMI Government Liability Act (essentially identical to the Federal Tort Claims Act).
* Reimbursement of up to $250 for pre-employment health screening costs.
* Annual Leave: Accrues at eight (8) hours per pay period (bi-weekly), with a full-time Employment Contract
* Sick Leave: Accrues at four (4) hours per pay period (bi-weekly)
* Up to 5 days of paid leave for CME.
