_About the Physician – Home Based Primary Care Medical Director position_

The Sheridan VA Health Care System (SVAHCS) is excited to announce an opportunity for a highly qualified physician to provide leadership to the Home Based Primary Care (HBPC) Program. The incumbent will develop and implement HBPC processes and procedures, participate in clinical activities, develop medical community partnerships, coordinate with PACTs, advise management of clinical and/or administrative issues, serve as a consultant to peers, subordinates and administration, as well as supervise physicians, physician assistants and advanced practice registered nurses. _About the Sheridan VA Health Care System_

We are part of the Rocky Mountain Region Veterans Integrated Service Network (VISN19). VISN 19 includes facilities in Colorado, Oklahoma, Montana, Utah and Wyoming. Our health care system consists of our main facility located in Sheridan, Wyoming. Its a 200-bed medical center, serving as the psychiatry tertiary referral site for VISN 19. We provide complex care across the continuum ranging from acute inpatient mental health care to mental health residential rehabilitation and interdisciplinary outpatient programs, and prospective candidates will help drive an integrated and comprehensive approach to the care of a complicated Veteran population. Sheridan VAHCS bed complement includes 20 psychiatric, 10 medical, 55 extended care, 115 mental health residential beds, and a 45-bed Domiciliary Residential Treatment program for homeless Veterans. We have five Community Based Outpatient Clinics located in Casper, Riverton, Cody, Gillette and Rock Springs, with two Telehealth Outreach Clinics in Worland and Evanston; and one Outreach Clinic in Afton, Wyoming.

Education & Experience Requirements:

Degree of Doctor of Medicine or an equivalent degree resulting from a course of education in allopathic medicine or osteopathic medicine. The degree must have been obtained from an institution whose accreditation was in place for the year in which the course of study was completed. Approved schools are:

1. Schools of medicine accredited by the Liaison Committee on Medical Education (LCME) for the year in which the degree was granted, or
2. Schools of osteopathic medicine approved by the Commission on Osteopathic College Accreditation of the American Osteopathic Association for the year in which the degree was granted.
3. For foreign medical graduates not covered in (1) or (2) above, facility officials must verify with the Educational Commission for Foreign Medical Graduates (ECFMG) that the applicant has met requirements for certification, and must obtain a copy of the ECFMG certificate, if claimed by the applicant. [If the applicant does not claim an ECFMG certificate, facility officials must still confirm that the medical school meets (or met) ECFMG eligibility requirements for the year the candidate graduated.]

Expected Salary range will be competitive for the academic rank and include a full benefits and features package to support high quality professional practice and favorable work-life-family balance.
