Team Lead Safe Haven IHH

Safe Haven is a program serving individuals with co-occurring mental health, substance use and often, homelessness. There are three components: a drop-in center, street outreach, and an Integrated Health Home team. CCA is searching for a Team Leader for the Integrated Health Home (IHH) team. The Team Leader must be an LCSW, LICSW, LMHC, or LCDP. Oversee the daily operations of a multi-disciplinary team comprised of a case manager, nurse, psychiatrist, and peer recovery support. Provide services in a trauma-informed, person centered, harm reduction approach. Must be competent in CBT, DBT, and MI. Strong computer skills. Skilled at completing treatment plans and DAP/SOAP progress notes. Able to multi-task. Strong skills in assessment and de-escalation. This is truly life-saving, life-changing work.
