Position Description

POSITION SUMMARY: Provide individual and group peer case management support services to adults living with co-occurring substance use and mental health disorders in order to maintain and improve their overall wellness and level of functioning. As a critical social determinant of health, emphasis is placed on identifying and resolving homelessness and housing insecurity. Services are provided in a manner that is trauma-informed and offered in accordance with harm reduction and housing first approaches.

EDUCATION AND TRAINING REQUIRED: Peer Recovery Specialist, Certified or working towards certification is encouraged, Bilingual (Spanish/ English) preferred. Ability to communicate effectively orally and written.

WORKING CONDITIONS: Consistently complete work in a competent, accurate, and thorough manner. Ability to be a self-starter, attends to multiple tasks simultaneously, work with minimal supervision, and continually seeks new/better methods to organize/complete work. Effectively accomplish duties by working with others, including team members, other CCA staff and/or outside agencies. Regularly available for work, reliable and on time for all things scheduled. Ability to coordinate visits with clients in a variety of locations: office, client home, community locations, including outdoors. Conduct outreach activities throughout the community. Demonstrate competency in clinical/technical or administrative skills, procedures, methods, and tools required to perform the job successfully.
