Community Care Alliance is actively seeking qualified candidates for a full-time Outpatient Crisis and Community Liaison Case Manager position in their Family Behavioral Health Department.
The Outpatient Crisis and Community Liaison Case Manager will provide crisis intervention to clients and families who are at risk due to behavioral health and social determinants of health, with a focus on traditionally underserved populations, including African American and Asian American. Services will take place in a variety of settings and can be intensive in nature. The Outpatient and Community Liaison Case Manager functions as a part of a team and are required to collaborate with other team members and individuals within our agency as well as with outside agencies and individuals involved with families.

Education/Licensing Requirements : Minimum Associate degree in Human Services, Bachelor’s in Social Work or related field preferred; Driver’s license.
