Job Summary and Qualifications

POSITION SUMMARY:
The Stroke Coordinator is responsible for organizing and administering the
stroke program. The Stroke Coordinator works collaboratively with patients and
their families, healthcare providers, EMS, and community groups to facilitate
Stroke Care and prevention throughout the continuum of care. The Stroke
Coordinator collaborates with staff to identify and lead advanced clinical
care for Stroke patients. Provides leadership and system planning to maintain
Disease-Specific Care Certification.
The Stroke Coordinator is responsible for maintaining appropriate education
and certification so as to be a resource and the clinical lead for the care
patients within the service line including but not limited to post
endovascular thrombectomy and alteplase patients, as well as neurosurgical and
neurointerventional radiology patients.
SPECIFIC ELEMENTS & ESSENTIAL FUNCTIONS:
Functions as a member of the healthcare team to facilitate and promote
teamwork toward promotion of quality patient care.
Initiates and leads educational activities for comprehensive stroke
requirements and participates in local health fairs and symposia. Attends
local, regional, and national meetings as needed and defined by Comprehensive
Stroke certification standards.
Conducts mock Code Drills during normal and off shift hours to ensure staff
preparedness.
Works to assure successful survey process outcomes for disease specific
certification; role requires flexibility in hours due to survey preparedness.
Responsible for facilitating and coordinating the key elements of the Stroke
Center Certification process including development and review of protocols and
policies and attendance at off hour meetings.
Educates staff and physicians on standards compliance and assessment /
treatment per evidence based practice for acute stroke patients. Collaborates
with physicians and organizes meetings offsite as needed to build
relationships and rapport for continued volume enhancements.
Provides education to staff and physicians to meet comprehensive stroke
requirements. Educational opportunities exist both during normal and off
hours.
Leads or participates actively in patient centered research related to stroke
per requirements of Comprehensive Stroke certification. Proficient at entire
research process including IRB.
Interacts with patients and families of acute stroke patients to determine
care protocol compliance and patient satisfaction.
Initiates and manages databases for recording and reporting of data related to
disease specific certification/accreditation.
Specialty Certification preferred, 2-3 years clinical experience in
neuroscience nursing, critical care experience including acute neurological
care in the emergency department or intensive care setting
Aggregates and reports on data relevant to certification programs in relevant
committees.
Develops written reports related to assessments and recommends change in the
clinical practice environment which may be required.
Utilizes and facilitates research that supports and advances nursing practice
which meets comprehensive stroke standard requirements.
Assists facility staff to achieve competencies around care of the stroke
patient; utilizes a multidisciplinary education plan to include all
disciplines who are involved in the care of the stroke patient including
physicians.
Works collaboratively with department directors to evaluate educational needs
of staff and develops and implements plans to meet those identified needs in
conjunction with the Professional Development department and unit Leadership
teams. Utilizes innovative and diverse educational methods to best meet the
needs of the staff.
Assists in assuring that appropriate standards of care, practice and
performance, and policy and procedures are in place through direct observation
of clinical practice and collaboration with the management team.
Communicates effectively, both written and verbally, with all members of the
healthcare team, customers and key stakeholders.
Assumes responsibility for own growth as a professional including
participation in a professional organization, attendance to professional
seminars and through formal educational activities.
Other duties as assigned.

# What qualifications you will need:

* Basic Cardiac Life Support must be obtained within 30 days of employment start date

  • Registered Nurse in the state of NH, or compact licensure
  • Bachelors degree
  • Specialty Certification preferred
  • 3+ years of related experience
  • 2-3 years clinical experience in neuroscience nursing, critical care experience including acute neurological care in the emergency department or intensive care setting
