Position Description

Job Summary:

The RN Clinical Practice Manager is proficient in all tasks performed by clinical staff members, additionally, the RN Clinical Practice Manager acts as a role model, trainer, and mentor for clinical staff. Works cooperatively with the RN’s, Clinical Practice Supervisor, and Clinical Director to coordinate and facilitate completion of work required to promote high levels of patient care and service. Performs duties and responsibilities in a manner consistent with our mission, values, and standards.

Essential Functions

* Promotes teamwork and enhances a positive work environment. Encourages, supports teamwork and mutual respect among all coworkers

* Possesses excellent customer service skills and acts as a role model and providing exceptional service

* Supervises the clinical supervisor and the nurse care coordinator

* Handles personnel issues with medical assistance and phlebotomists including training and evaluating performance of their clinical team members

* Coordinates with LPN’s, RN’s, Clinical practice supervisors, and flow coordinators to identify and address opportunities to provide better patient care and services

* Perform periodic and timely clinical competency evaluations for LPN’s, RN, Clinical practice supervisor flow coordinator and certified medical assistant

* Determines work priorities and assigns work to ensure timely workflow for patient care

* Provides directions and monitoring of medication administration by medical assistants

* Assist providers with special tests and procedures within the standard scope of practice

* Assist in center with patient care as needed including but not limited to rooming patients’ medication refills referrals and phone calls

* Coordinates with providers to determine eligibility of patients for controlled substance case management. Responsible for ensuring medication refills are completed patients are compliant with medications inappropriate follow up is completed

* Completes medical prior authorization as needed

* Overseas the referral process to ensure provider referral buckets are managed timely with appropriate care coordination

* Assume self-responsibility for all direct patient care activities as indicated by the clinical director within the scope of the RN license

* Makes operational decisions as appropriate in the absence of the clinical director

* Manages the practice vaccine program and assumes responsibility for compliance reporting staff training and ordering of all vaccines and supplies

Experience and Skills:

Education and Experience:

* Licensed and credentialed Registered Nurse in the state of Pennsylvania

* Three to five years’ experience in clinical or community resource settings

* Care coordination and/or case management experience is strongly desirable

* Evidence of essential leadership, communication, education, and counseling skills

* Proficiency in communication technologies: i.e., e-mail, cell phone, etc.

* Highly organized with ability to keep accurate notes and records

* Experience with health IT systems and reports is highly desirable

* Local knowledge about connections to community health care and social welfare
