Job Summary and Qualifications

The Advanced Clinical Pharmacist is responsible for providing all levels of
pharmacy clinical services. This includes monitoring patient medication
regimens and providing appropriate interventions as well as supporting all
aspects of pharmacy operations. This position does require close interactions
with physicians/nursing colleagues.

# What qualifications you will need:

* Current license from the Texas State Board of Pharmacy or must be licensed by start date if relocating

• Doctorate of Pharmacy from accredited school of Pharmacy required
• Completion of PGY-1 plus 3-5 years in Solid Organ Transplant required or completed/ in the process of completing PGY-2 in Solid Organ Transplant from ASHP accredited
• Specialty BPS certification preferred
• FCCP, FASHP preferred
• ACLS/BLS preferred
