APRN – Delta Regional Transplant Liaison

Summary of Job Duties:

We are seeking a dynamic and experienced Advanced Practice Registered Nurse (APRN) to join our team as the Delta Regional Transplant Liaison. This clinical APRN will be a designated member of our transplant team, responsible for coordinating the clinical aspects of care. As the Delta Regional Transplant Liaison, you will work closely and collaboratively with patients, their families, and other team members, including physicians, nurses, social workers, dietitians, and financial coordinators. Your primary responsibilities will involve directly overseeing and coordinating the care of transplant patients both during and after their procedures.

Your role will be multifaceted, requiring both clinical skills and interpersonal skills. You will ensure the smooth and efficient delivery of care, while also providing compassionate and tactful support to patients and their families during this challenging time. In addition to your clinical responsibilities, you will play a vital role in organizing and managing the care of transplant patients outside of the hospital. This includes acting as the liaison between the transplant team and patients’ healthcare providers, ensuring a smooth transition between care settings and maintaining continuity of care. You will actively participate in quality and performance improvement processes, contributing to ongoing program development and improvement. You will assist in providing education to patients, families, and staff regarding the transplant and associated plan of care.

Qualifications:

Minimum Qualifications:

Active APRN license in good standing.

Bachelor’s degree in nursing (BSN) or higher, required.

Master’s degree in nursing or a related field, preferred.

Certification as an Advanced Practice Registered Nurse (APRN), Adult or Family, required.

Two (2) years of Nursing experience.

Preferred Qualifications:

Care Coordination and Transition management or Outreach/multi-disciplinary clinical education.

Knowledge, Skills, and Abilities:

Knowledge of transplant protocols, procedures, and best practices.

Strong communication and interpersonal skills, with the ability to effectively collaborate with multidisciplinary teams.

Proficiency in electronic medical record (EMR) systems.

Ability to work independently and collaboratively in a fast-paced environment.

Strong attention to detail.

Compassion and empathy towards patients, families, and staff.

Ability to prioritize tasks and handle multiple responsibilities simultaneously.

Additional Information:

Key Responsibilities:

Establishes rapport and a relationship with the patient and family in order to understand their needs and expectations and to assist them in setting realistic and mutual goals. Integrate an awareness of cultural factors in the patient/family interview process and elicit clinically relevant cultural information.

In conjunction with the physician, the patient and interdisciplinary team, establishes a comprehensive plan of care to appropriately address clinical milestones.

Communicates plan of care, including changes and issues related to plan of care to patient/family, physicians, and other members of the healthcare team. Facilitates communication between the physician, interdisciplinary team, patient, and family.

Gathers sufficient information from all relevant sources to determine the effectiveness of the plan of care to ensure it is done in an accurate, safe, timely, and cost-effective manner.

Organizes, integrates, and evaluates the effectiveness of the plan of care and progress toward achievement of desired outcomes.

Coordinates patient transfer to the appropriate level of care. Identifies and facilitates resolution of clinical and operational roadblocks to achieve optimal outcomes by identifying alternatives as needed.

The APN will travel to dialysis centers and regional hospitals as an outreach educator and liaison to local providers for transplant education and community support for transplant patients.
