Position Description

What You Will Do:

We are seeking a collaborative, team-oriented, and creative individual to provide substance use and mental health screening, brief intervention, brief therapy/psychotherapy (for those whose scope is therapy/social work), and referral to services for adult patients at a Family Practice location in West Virginia. The Clinician will work with a multidisciplinary team of professionals, including physicians, nurse practitioners, medical assistants, nurses, front office staff, and off-site behavioral health staff. At the administrative level, this role will involve working to support behavioral health integration in a Family Practice and evaluation of integration efforts. Most of the Clinician’s time will be spent in patient service delivery. Specifically, the Clinician will assess patients’ behavioral health/substance use needs through screening, deliver brief motivationally informed interventions to promote healthy behavior change, offer time-limited skills-based psychotherapy (45-50 minutes sessions, up to 12-15 sessions–only for those whose scope is counseling/therapy/social work) for patients in need of additional support in changing their substance use behavior and/or coping with other mental health concerns, and connect patients who need a higher level of behavioral health care with local facilities for further treatment. This position is funded by a Health Resources and Services Administration (HRSA) grant, and as such, entails grant-required trainings.
Responsibilities and Duties
Responsible for performing necessary functions of the integrated care position including screening, brief intervention, brief treatment, and referral to treatment in accordance with clinic guidelines, protocols, and standing orders. Collaborate with other medical and behavioral health professionals to develop, implement, audit, and modify procedures and services for behavioral health. Facilitate screenings of all eligible medical patients 18 years and older. Deliver brief intervention and brief therapy for medical patients including, but not limited to, utilizing motivational interviewing and behavioral and cognitive behavioral therapy techniques in standard therapy appointment slots (for those whose scope includes counseling/therapy/social work). Support psychotropic medication management as prescribed by medical providers, focusing on treatment adherence and monitoring, side effects, and effectiveness of treatment. Participate in regularly scheduled (usually weekly) caseload consultation with the psychiatric consultant and communicate resulting treatment recommendations to the patient’s medical provider. Consultations will focus on patients new to the caseload and those who are not improving as expected under the current treatment plan. Case reviews may be done by telephone, video, or in person. Complete referrals for higher levels of care to behavioral health centers/providers and track responses to referrals. Facilitate referrals for clinically indicated services outside of the organization (e.g., social services such as housing assistance, vocational rehabilitation, mental health specialty care, substance abuse treatment). Develop and complete relapse prevention self-management plan with patients who have achieved their treatment goals and are soon to be discharged from the caseload. Ensure all patient records, charts, and related documentation is current and consistent with grant requirements. Facilitate and coordinate data collection and transmission efforts in conjunction with Collaborative Care project team. Assists in compilation of data and patient outcomes as requested.

What You Will Bring:

Education
Masters in Behavioral Health Field, Social Work/Counseling or Bachelors in Nursing (BSN)

Experience
Experience working in medical setting preferred, for those with their LICSW or LPC.
Experience working with behavioral health patients for those who have their RN.

Certification & Licensures
West Virginia-Licensed Clinical Social Worker (LICSW), West Virginia Licensed Professional Counselor, or West Virginia Registered Nurse preferred

Qualifications
Demonstrated capacity to support models of substance abuse/behavioral health integration into primary care required
Familiarity with local resources and their programs required
Prior academic or practical training in motivational interviewing and cognitive-behavioral therapy required
Excellent written and verbal communication skills required
