Position Description

_This FQHC has an opening for a LMSW/LCSW/LPC who would like to join an established and stable community health center focused on providing quality patient care._

JOB OBJECTIVES:

1. Work cooperatively with MHHC’s providers and personnel to carry out the goals and objectives of MHHC, according to established policies and procedures.
2. Will treat others respectfully whether it is a patient, fellow employee, member of management, customer, vendor or visitor to our premises.

3. Assess, diagnose, and evaluate clinical symptoms, developmental and personality disorders, severity of psychosocial stressors and global functioning and address medical conditions as they affect patients’ functioning.

4. Plan treatment based on the individual client’s needs and resources and on clinic and community resources.

5. Coordinate professionals involved in patient care to insure interdisciplinary collaboration and continuity of services.

6. Provide comprehensive and current knowledge of community resources for supplementary treatment of clients and make referrals, which adequately and appropriately utilize these resources. Is available for consultation with other clinic staff regarding community resources.

7. Provide a continuum of short- and long-term psychotherapy services to individuals, couples, families, and groups, based on individualized treatment plans.

8. Function as an integral part of a multidisciplinary department including psychiatrists, psychologists and clinical nurses.

9. Maintain good communication and collaborative relationships with referring physicians and other staff and recommend additional referrals for patients as needed.

10. Work with collateral personnel as needed in the planning and delivery of services and case management.

11. Carry out ongoing record keeping and case review of all psychotherapy work in compliance with State guidelines.

12. Carry out all above-mentioned duties with the highest professional standards, including maintenance of patient confidentiality.

13. Provide assessment (by telephone and in person) of dangerousness and immediacy of crisis situations.

14. Design and carry out appropriate interventions in crisis situations.

15. Offer in-service education programs and functions as a resource to the medical staff and the community as needed.

16. Participate in development and selection of patient education materials and services.

17. Represent the Health Center and the Counseling Staff to the community at large in a professional and helpful manner.

18. Maintain pertinent patient information in the medical record.

19. Complete the necessary written information and data in a timely fashion.

20. Must comply with the provisions of the Health Insurance Portability and Accountability Act (HIPAA) of 1996.

ESSENTAL JOB FUNCTIONS:

1. Assess and treat a wide variety of patients/clients with accuracy, proficiency, and efficiency at an advanced professional level.

2. Cognitively read and interpret a variety of instructions furnished in written or oral form.

3. Effectively present information and respond to questions from patients, and other individuals encountered in the course of employment.

4. Communicate with patients and office staff with tact and patience utilizing good communication skills.

5. Must comply with the provisions of the Health Insurance Portability and Accountability Act (HIPAA) of 1996.

6. Perform effectively as a member of a multidisciplinary team.

7. Clean and neat appearance.

Education & Experience Requirements:

Master’s Degree in a mental health field with documented clinical course work and field experience. Possess broad background of training and experience with various patient populations and modes of service delivery.

Weekdays (8-5)

Compensation:

salary is based on experience.

Compensation Range:

$50,000 – $100,000
