Position Description: Heartland Health Center (HHC) is a Federally Qualified Health Center (FQHC) dedicated to providing affordable and comprehensive primary healthcare and dental services to uninsured, under-insured, and low-income individuals and families in Grand Island and the surrounding communities. The behavioral health specialist delivers counseling services in the primary care clinic setting to a variety of age groups. The behavioral health specialist will utilize an integrated healthcare model and collaborative care to work collaboratively with primary care providers in addressing patients’ whole health, and also works closely in alignment with the mission, values, and objectives of the department and organization. Responsibilities: * Conduct regular therapy appointments with patients in the clinic, structuring each session to accomplish the goals and objectives stated in the treatment plan; evaluate the outcome of treatment according to the goals and objectives sought. * Embrace a patient-centered, integrated healthcare model, collaborating with healthcare providers from a variety of disciplines. * Address all aspects of patient health by consulting with patients and primary care providers during medical appointments when needed. * Coordinate services with outside referral sources to facilitate successful continuum of services. * Ensure that client documentation requirements are completed in a thorough and timely manner. * Represent ethical interests of mental health services and adhere to best practices required for compliance and exemplary patient care. * Adhere to departmental goals, plans, and standards consistent with the administrative, legal and ethical requirements/objectives of the organization. * Attend to additional tasks when assigned by supervisor. * Provide brief behavioral interventions via phone and in person for an assigned caseload using evidence-based techniques such as behavioral activation, goal setting, talk treatment, symptom monitoring, problem-solving treatment, motivational interviewing, psychoeducation, or other treatments as appropriate. * Screen and assess patients for common mental health and substance use disorders. Facilitate patient engagement and follow-up care. * Support psychotropic medication management as prescribed by medical providers through collaboration with the patient’s psychiatrist, focusing on treatment adherence monitoring, side effects, and effectiveness of treatment. * Participate in weekly caseload conferencing with the psychiatrist and communicate resulting treatment recommendations to the patient’s medical provider. Case conferencing prioritizes patients new to the caseload and those who are not improving as expected. * Systematically track treatment response and monitor patients (in person or by telephone) for changes in clinical symptoms and treatment side effects or complications. Document in-person and telephone encounters in a patient registry and electronic health record, while focusing on identifying and re-engaging with patients who need it the most. Effective documentation allows for sharing and collaborating among the entire treatment team. * Demonstrate skillful assessment of risk of harm and safety planning when needed. Facilitate referrals for clinically indicates services outside of the clinic, such as mental health specialty care or substance abuse treatment. * Develop and complete patient-managed relapse prevention plans with patients who have achieved their treatment goals and are soon to be discharged from the caseload. Qualifications: * Master’s degree in Social work or therapeutic mental health (counseling, marriage and family therapy, etc.) required. * Provisionally or Fully licensed as a Licensed Clinical Social Worker (LCSW), Mental Health Practitioner (LMHP/PLMHP), or Licensed Independent Mental Health Practitioner (LIMHP). * Training and/or experience in evidence-based mental health interventions. * Demonstrated ability to work effectively with culturally diverse patients and staff. * Demonstrated comfort and ability to work with patients across the life cycle. * Demonstrated flexibility and adaptability to working in a fast-paced clinic, all while maintaining accurate patient documentation in a timely manner. * Prior experience with an Electronic Health Record (EHR) program preferred. * Strong computer skills in Microsoft Office products required.
