POSITION SUMMARY:
Under the direct supervision of the Director of Ancillary Services, the HIV Medical Case Manager is responsible for the coordination and implementation of HIV case management plans. The Medical Case Manager will also perform outreach and patient advocacy as needed. All tasks to be performed per NHSC protocol unless stipulated otherwise by NHSC contractual agreement with funding source.
ESSENTIAL FUNCTIONS and PRINCIPAL ACCOUNTABILITIES:
A. Complete a psychosocial assessment on all NHSC’s Ryan White patients.
B. Document specific patient needs and plans for addressing needs. Reassess patients’ status every six months or as needed.
C. Develop and implement medical case management treatment plans for comprehensive services.
D. Monitor patients’ compliance with HIV clinical care and support services, medication and treatment adherence, and other relevant appointments.
E. Contact non-compliant patients and assess needs and barriers to obtaining services.
F. Participate in interdepartmental case conferences as needed.
G. Serve as liaison between patients, NHSC and other community services at the request of the supervisor.
H. Advocate on behalf of patients in obtaining needed services and resources.
I. Complete initial assessment and periodic reassessments as required and update progress notes after every patient encounter.
J. Follow reporting and client contact guidelines as required by NHSC and funding agreement.
POSITION QUALIFICATIONS
Education Required
Education or Formal Training: master’s degree preferred, or bachelor’s degree (with additional experience), preferably in human services, or RN prepared. Case Management experience. Bilingual (English/Spanish) preferred.
License, Certificate Registration
New Jersey HIV Counselor certification or be eligible for one.
Previous Work Experience
Experience working with HIV and/or other vulnerable patient/client populations, and strong administrative skills.
Special Skills or Knowledge
Ability to make sound decisions and to establish and maintain effective working relationships with staff associates and the public. Excellent communications, counseling, and interviewing skills necessary. Excellent written communication skills required. Thorough knowledge of the community and population specifics.
