General Duties:
Managing a caseload taken from population of individuals that meet clinical requirements for Chronic Care Management (CCM)
Work closely with clinical support staff, nursing leadership, physicians, and other medical providers to achieve system goals of improving clinical outcomes for patients with chronic diseases
Developing processes that support the CCM program
Ensuring all requirements have been met in order to bill and receive reimbursements that support the program for continued growth
Hours: 8:00am-5:00pm (Clinic Hours) Monday-Friday
Requirements:
Must have knowledge of insurance industry practices and requirements
Must have an understanding of chronic disease prevention care measures
Must hold a licensure as a Registered Nurse
Experience working with patients with mental health issues is preferred.
Must be able to read, analyze, interpret and comprehend instructions, correspondence, professional journals, and technical procedures
Strong communication skills
Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and other routine mathematical functions.
Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions and deal with several abstract and concrete variables. Ability to apply common sense, in difficult situations.
Ability to establish and maintain an effective working relationship with team members, staff, patients, and other third parties.
Ability to operate computer, keyboard, and printer. Knowledge of how to find and ability to interpret the information given from the EHR system, nursing staff, physicians and medical providers, to properly diagnose injuries, diseases, and deformities including treatment alternatives, drug properties and interactions, and preventive healthcare measures.
Ability to concentrate under high demands and listen effectively. Must have good judgment and problem-solving skills. Knowledge of ICD-9 and CPT coding.
Knowledge of health insurance procedures. Ability to organize tasks, work as a team member and/or independently using sound case management skills with a variety of professional disciplines.
