Job Summary and Qualifications

The Clinical Rehabilitation Specialist contributes to high-quality, patient-
centered care by identifying candidates for inpatient rehabilitation and
initializing timely, appropriate patient transfers. In collaboration with the
Rehab Medical Director, the Clinical Rehabilitation Specialist ensures
appropriate patient placement through clinical assessments and screening. The
Clinical Rehabilitation Specialist serves as an inpatient rehabilitation
expert between internal and external referral sources and inpatient
rehabilitation operations. The Clinical Rehabilitation Specialist is
responsible for establishing relationships with case managers, physicians,
payors, and other referral sources.
What you will do in this role:

  • You will assist and coordinate intake and pre-admission screening processes. You will document evidence of rehab admission criteria within established timeframe.
  • You will perform on-site and off-site clinical evaluations to assist Rehab Medical Director in establishing patient eligibility for admission.
  • You will provide information to patient/family/caregiver related to inpatient rehab and program features. You will communicate expectations and requirements for participation in the rehab program and conduct or coordinate tours of the rehab unit.
  • You will obtain information relating to patient resources and healthcare insurance benefits.
  • You will demonstrate clinical knowledge of rehab admission criteria to appropriately identify and qualify rehab patients.
# What qualifications you will need:

* Basic Cardiac Life Support must be obtained within 30 days of employment start date

  • Associate Degree
  • Licensed Physical Therapist, or Occupational Therapy (OT), or Registered Nurse, or Speech Therapist
