Family Practice Physician

Board certified and/or eligible Family Practice Doctor of Medicine

Successful completion of internship and residency

Current, valid Indiana medical license

Knowledge of health systems, preventive medicine concepts, practices, methods and techniques of general medicine

Knowledge of state and federal laws regulating medical profession

Admission privileges at area hospital

Able to train, supervise, counsel and direct professional and technical personnel

Effective oral and written communication skills

Scope of Tasks & Responsibilities:

Provide specialized medical service which includes primary care and urgent/acute care for all age groups.

Ensure continuity of patient care through health maintenance activities including evaluation, early detection and management of problems

Provide gynecologic and obstetrical care to include prenatal care, hospital delivery and post-partum care

May maintain database and statistics to monitor care provided

Provide after hours call coverage according to call schedule

Provide supervision and training for students and residents assigned to RHC

Collaborate with all providers including physicians and mid-levels – both RHC staff as well as volunteers.

Maintain effective relationship with the Indiana University Family Practice Residency Program

Attend regularly scheduled meetings.

Commit to and ensure work is operating in a manner consistent to the Raphael Health Center Mission

Treat patients, visitors and coworkers with respect and dignity

Maintain timely electronic medical records according to health center policy and meaningful use guidelines.Meaningful use requires that, upon request, patients are provided an electronic copy of their health information (including diagnostic test results, problem list, medication lists, medication allergies) within three (3) business days; and are provided a clinical summary for each office visit within three (3) business days.
