Position Description

Are you a dedicated and compassionate licensed mental health provider ready to make a real impact in a community health setting? We are seeking a skilledBehavioral Health Consultant (BHC) to provide integrated care and improve health outcomes for our patients. As part of our close-knit team you’ll be directly contributing to the health and well-being of individuals in our community.

As a Behavioral Health Consultant, you’ll play a vital role in integrating behavioral health into primary care services. You will provide essential behavioral health consultations, support patient care teams, and deliver evidence-based interventions that enhance patients’ overall health.

Key Responsibilities:

* Provide Brief Consultations: Conduct individual/family consultations, psychoeducation, motivational interviewing, and cognitive behavioral interventions.

* Coordinate Care: Collaborate with primary care providers to improve patient health and provide referrals for specialized services.

* Crisis Intervention: Respond to behavioral health emergencies during clinic hours.

* Consult and Advise: Offer guidance to healthcare providers regarding behavioral health matters, treatment planning, and patient care.

* Documentation: Maintain accurate assessments, progress notes, and treatment plans.

Required Qualifications:

* Licensed Masters Social Worker (LMSW) required, Licensed Specialist Clinical Social Worker (LSCSW) or Licensed Clinical Psychologist preferred

* Experience in a medical or behavioral health setting

* Must pass a background check

* Familiarity with integrated behavioral health models

* Proficiency in solution-focused behavioral interventions

* Ability to manage crises and provide on-the-spot support
