Asian Services In Action’s International Community Health Center (ASIA-ICHC is a federally qualified health center . ASIA-ICHC predominantly serves low-income, underserved, limited English-speaking Asian American and Pacific Islander (AAPI) and other immigrant and refugee populations of all ages. Our mission is to empower AAPI’s in Northeast Ohio to enhance their well-being, build their communities and to improve their self-sufficiency through culturally and linguistically relevant information and services.

ASIA-ICHC provides medical and psychological services and is looking for an experienced licensed social worker for our integrated care clinics in the Akron and Cleveland locations. ASIA-ICHC is looking for a candidate who is a self-starter, reliable and efficient in dealing with client’s behavioral challenges and addressing social determinants of health. Independent licensed individuals are preferred, but not required. Bonus: Student Loan Forgiveness; Supervision with LISW-S.
POSITION SUMMARY:
Provide clinical behavioral health counseling for mental health and substance use disorders
Assist with healthcare integration initiatives of general and behavioral healthcare, to ensure that patients are provided with or connected to services including AOD, case management and clinical care
Make internal and/or external referrals to meet patients’ social needs
Provide umbrella supervision and guidance for social work interns
Assist within language abilities behavioral outreach program to the linguistic minority community
ESSENTIAL DUTIES AND RESPONSIBILITES:
Provide clinical mental health services: mental health assessment, individual counseling, and group counseling as needed.
Provide substance use assessment and intervention services
Provide clinical supervision and support for social work interns.
Provide professional training in individual, group, and family counseling, crisis intervention and emergency management with appropriate involvement of psychologist, psychiatry provider.
Provide case management functions to ensure quality and outcome of patient care;
Serve as a liaison between ICHC and other departments within ASIA Inc. to promote organization integration.
Ongoing updating and utilization of the client referral system to better serve ICHC’s patients’ needs including integration with ASIA social service department and community resource database.
Consult with professional staff, medical providers and nurses regarding ongoing or acute mental health issues. Review referrals to determine patient needs.
Assure compliance with regulatory requirements, including maintenance and accuracy of required clinical records.
Outreaching to ICHC patients and community members to provide education and awareness
Maintain all client information in a confidential manner in accordance with policy and HIPAA regulations.
Work as part of the team to promote excellent patient experience and seamless patient care that is consistent with ICHC / ASIA values.
QUALIFICATIONS:
Current LISW/LPCC/LMFT to practice in the State of Ohio.
Master’s degree is preferred and will consider lower level of license (eg. CDCA, bachelor level LSW/) with at least five years of experiences in substance abuse treatment
Working knowledge of and ability to further implement integrated and collaborative care between Behavioral Health and Physical Health in a Primary Care setting and in community behavioral health systems
Demonstrated ability to identify and effectively utilize resources
Demonstrated leadership skills, computer proficiency, in-depth written and oral communication skills, decision making skills; demonstrated ability to recognize alternatives, evaluate relevant information and make timely decisions
In-depth emotional intelligence and ability to achieve desired results through collaboration with strong team leadership and membership skills
Openness to working with diverse populations with a lens of cultural humility as well as a trauma informed and strength based approach. Further, to work with individuals and groups including those within linguistic minorities, refugee population, and limited income
Familiar with community resources in Summit County and/or Cuyahoga County
Working in the field of social service or health care as a behavioral health provider and/or case manager
Speaks a second language (such as Nepali, Chinese, Swahili, etc.) is preferred
