Position Description
Consideration for a raise after successful completion of 90-day introductory period.
$5,000 sign on bonus (paid out in installments at 6 months, 9 months, 1 year)

_SPECIFIC RESPONSIBILITIES:_
· In coordination with the CEO and Program Administrator may conduct perspective resident screening interviews and documentation reviews.
· In coordination with the Program Administrator conducts the initial Comprehensive Intake Assessment with residents.
· Develops Service / Treatment Plans for residents outlining specific program goals and objectives.
· In cooperation with nursing staff facilitates weekly Treatment Team meetings.
· Provides resource information and referrals to assist residents in developing community support systems such as developing supportive relationships, self-advocacy, stable housing, education, and employment.
· Regularly monitors resident Service / Treatment Plans and updates accordingly.
· Conducts individual and group therapy sessions as assigned
· Creates individualized crisis plans for residents.
· Provides crisis intervention to residents as needed during crisis periods.
· Monitors residents to ensure their safety within the facility.
· Observes and document behavioral changes in residents.
· Acts as a liaison with medical professionals and behavioral health professionals providing services to residents.
· Responsible for accurate and timely charting in ECP using accepted abbreviations and terminology.
_QUALIFICATIONS, EXPERIENCE, EDUCATION and LICENSURE: _
· A Master’s degree in Counseling, Social Work, or comparable degree.
· Active Washington State Licensure, preferred
· Meets health requirements, i.e. – flu vaccination, COVID-19 vaccination and current booster, TB testing – per licensing requirements.
· Some prior residential facility experience desirable.
· CPR/First Aid Certified.
· Valid Washington State Driver’s License required or the equivalent if residence in Washington is new.
· Experience using word processing and spreadsheet software. Microsoft Word and Excel preferred.
_TRANSPORATION:_
· Uses personal vehicle to travel to agency sites and non-agency locations.
· May drive residents to appointments and outings using agency vehicles.
_COMMUNICATION:_
· Leads and participates in staff meetings and in-services training.
· Consults with management staff, nursing staff or peers when knowledge base in a given area needs to be expanded.
· Requests literature, in-service or orientation on a given topic.
· Demonstrates accountability regarding equipment and supplies.
_LANGUAGE SKILLS:_
· Ability to read, research, analyze, and interpret common scientific and technical journals, clinical information, and regulatory documents.
· Ability to respond to common inquiries or complaints from customers, regulatory agencies, or members of the business community.
· Ability to effectively present information to management, public groups and/or boards of directors.
· Proficient in English.

· Most the employee’s job duties are performed indoors in a residential setting. In some circumstances, the employee may be required to perform some job duties outside, in medical settings, and in the presence of non-employees, such as patients, families and state officials. There is a possibility of exposure to disease and contagions that could cause a cold, flu, COVID-19, and other illnesses. The noise level in the work environment is usually moderate.

Supreme Living provides comprehensive, cost-effective, and quality care to individuals requiring assistance in day-to-day living functions in home-like environments.
