Position Description
Our team of healthcare professionals positively impacts the health, well-being and quality of life for those we serve providing comprehensive, integrated, affordable healthcare and education to patients. We utilize a team-based model including embedded Behavioral Health Clinicians as well as Case Managers, Care Coordinators and Community Health Workers.
* Provides care and educates patients and families on the necessary lifestyle changes involved in a diabetes diagnosis, including medication, physical activity, glucose testing, and nutritional counseling (in the absence of the nutritionist). This is a part-time position with hybrid flexibility.
* Provides patient consultation for management of their diabetes. Monitors treatment protocols in conjunction with the patient’s primary care provider.
* Assess, monitor and implement treatment plans for patients at various stages of the disease with a focus on prevention of complications and the management of stable diabetes.
* Provide ongoing assessment and recommendations to the treatment team and patient/family for adjustments in the plan of care as required.
* Document assessments, interventions and plans including recommendations and goals, progress notes and discharge summaries.
* Communicates with Amoskeag Health provider staff about patient education sessions and progress made by patients.
* Teaches proper use of health care system and community resources.
* Instruct patients on insulin use, injection process and monitoring of blood sugar levels.
* Educates patients on coping strategies, psychological adjustment and family involvement and provides support pertaining to their diabetes.
* Teaches the importance of consistent exercise.
* Counsels patients on the acute and chronic complications of hyperglycemia and hypoglycemia.
* Updates list of community resources that would assist our clients in the management of their disease.
* Attends and participates in Amoskeag Health meetings as directed. If absent, responsible for reading minutes of meetings and signing the documentation. Attends mandatory safety-related and infection control in-service trainings as designated by the Senior Management Team.
* Participates in Process Improvement activities and is actively involved in Total Quality Management for Amoskeag Health, as needed.
* Displays sensitivity to cultural diversity of population being served. Maintains client age-related competence.

Education & Experience Requirements:
Qualifications:
* Education/Training: Master’s degree and current New Hampshire Nurse Practitioner license.
* Experience: One to five years’ experience as a Nurse Practitioner in a multi-disciplinary team providing Diabetic care and education to medically indigent families required.
* Certified Diabetic Educator preferred.
* BLS certification required.
* Experience in utilization of EMR.
* Prefer experience in an ambulatory health care environment.
* Ability to communicate clearly, ability to use various teaching approaches to meet the needs of different learning styles, knowledge of medical assistant clinical core functions. Change management training and experience desirable.

Weekdays (8-5)
Compensation:
Compensation: The starting rate for this position is $45.30 an hour, which reflects the beginning of the pay scale. At the time of an offer, determination of your rate will reflect your skills and experience as it relates to the position.
Compensation Range:
$50,000 – $100,000
