Nurse Practitioner- Family Medicine

Purpose: Provides patient-centered coordinated care model demonstrated through the following competencies: leadership, direct clinical practice, consultation/collaboration, coaching/guiding, research, and ethical decision making. The APN practices within the scope of practice as established by the APN., The NJ Board of Medical Examiners, and Alliance guidelines.
Reports To : Medical Director
Duties and Responsibilities include the following:

1. Provides culturally sensitive family medicine care predicated on evidence-based guidelines.
2. Participates in a team approach to Family Medicine, being cognizant of and responsive to the expertise and contributions of all staff.
3. Makes appropriate referrals.
4. Reviews all laboratory results as they are received from laboratory in a timely fashion as outlined in the lab follow-up policy.
5. With appropriate medical consultation and medically approved standing orders, provides comprehensive care to uncomplicated clients throughout their plan of care, including, but not limited to, the following:
1. Is compliant with the agency’s policies regarding timeliness of closing the visit encounter.
2. Is complaint with the agency’s policies regarding Infection Control.
6. Attends staff meetings as defined by the Management.
7. Maintains continuing education as outlined by APN and/or the NJ Board of Medical Examiners
8. Participates in the departments on call rotation schedule.
9. Maintains membership and participation in professional organizations.
10. All other duties as assigned.

Education & Experience Requirements:

Education and/or Experience – Degree required from an accredited nursing educational program or three to five years related experience and/or training; or equivalent combination of education and experience may be considered.
License/Credentials – NJ State Board of Medical Examiners Advance Practice Nurse License with prescriptive privileges; NJ Professional RN License, ACMB Certification as APN
