Position Description

Copper Queen Community Hospital Rural Health Clinics has an opportunity for a Nurse Practitioner / Physician Assistant to join our group in Douglas, AZ. We provide services for 60,000+ people within our 5 clinics, which include Laboratory, Radiology, and Physical Therapy.
Our benefits and compensation will include the following:

– Guaranteed salary of $100,725.73 plus based on experience with incentive for productivity bonus.
– Sign on Bonus $15,000
– Relocation Assistance up to $15,000
– Coverage under the hospital malpractice insurance
– $4,500 and 1 week paid for CME annually
– 10 paid holidays
– 15 days of paid vacation per year
– 120 paid sick hours per year with max accrual of 220 hours

Health Insurance | Effective the first of the month after date of hire. Paid 100% by hospital for employee and family. (Children and spouse are covered once employee coverage has been established.) |
Dental Insurance | Eligible after six (6) months of Medical Coverage. Paid 100% by hospital for employee and family. |
Vision Insurance | Eligible after six (6) months of Medical Coverage. Paid 100% by hospital for employee and family. |
Life Insurance | One and ½ times of salary plus $5,000. Effective date of hire. Paid 100% by hospital. |
Retirement Plan | 3% of salary less vacation and sick pay CQCH Contribution after 3 years from date of hire—3 years vesting. |

CME – Five (5) paid days per contract year to attend CME. Hospital will pay for reasonable expenses related to CME incurred by the Employee for travel, lodging and registration up to a maximum of $4,500 per year. CME hours and dollars cannot be accrued year to year. Employee must obtain CEO approval for CME courses and scheduled dates of CME prior to registering for CME courses. Original receipts will be required for reimbursement of expenses.

_Benefit Outline_

Employee shall receive benefits available to other full time Hospital employees subject to the same requirements and restrictions as those imposed on other employees. Such benefits are subject to change in accordance with Hospital policy. Employee’s Benefits include, without limitation, but subject to change:

1. Participation in Hospital’s employee retirement program
2. Fifteen (15) days (120) hours of vacation per year. CEO approval is required prior to taking vacation.
3. Ten (10) days of sick leave.
4. Ten (10) paid holidays.
5. Non-contributory health insurance
6. Workers’ Compensation Insurance
7. Arizona medical license fee, DEA license fee, CQCH Medical staff sues and certification fees for certificates/licenses related to ED services (e.g., ATLS and ACLS)
8. Five (5) paid days per contract year to attend CME. Hospital will pay for reasonable expenses related to CME incurred by the Employee for travel, lodging and registration up to a maximum of $4,500 per year.
