Position Description

## Physical Therapist

#### Full-time (40 hours/week), weekday shifts.
_This position qualifies for a $10,000 sign-on bonus and up to $25,000 in education loan repayment for qualifying student loans! _

With advanced hands-on therapy techniques, programs, and equipment, our therapists promote the health, independence, and quality of life of patients. Our therapists provide high-quality, comprehensive care to outpatients and inpatients alike, with the majority of services being provided through outpatient care.

Therapists are responsible for providing evaluation, treatment, and home instructions for patients and family members involved in the patient’s treatment. Therapy typically includes a combination of manual, hands-on treatment and exercises specifically designed for the patient’s injury, illness, or condition.

Requirements

Must be a graduate of an accredited Physical Therapy program. Must hold a current licensure with the ND Board of Physical Therapy.

Therapists must have knowledge of writing and dictating patient progress notes. Compliance with patient billing and insurance policies is required. Therapists must complete initial certification and progress notes in a timely manner. Must possess excellent communication skills in order to explain/direct home programs and exercises. Must be able to interact with patients and families in a positive manner.

Essential Functions, Roles, and Responsibilities

1. Performs physical therapy evaluations and treatments using safe and proper techniques.
2. Sets up and instructs patients in rehabilitation programs including designing and progressing home programs.
3. Completes certification and progress notes according to guidelines including assessment, goals, plan of care, frequency, and duration of treatment.
4. Must collaborate with multidisciplinary staff.
5. Establishes and instructs on a home program for each patient, revising the Plan of Care as necessary.
6. Accurately and effectively documents patient evaluations including short and long-term goals, rehabilitation potential, treatment plan and direction, and frequency of treatment.
7. Completes Clinical and Progress Notes according to guidelines, including Evaluation, Care Plan, Exit Instructions, Discharge Summaries, and OASIS forms.

Other Duties

1. Complies with state, Medicare, and insurance rules and regulations.
2. Maintains professional standards by participating in inservices, committees, and attending one workshop each year.
3. Establishes body mechanics – lifting classes for SMC staff and work site analysis.
4. Advises and consults with patients and patients’ families and other home care staff members.

Education & Experience Requirements:

Weekdays (8-5)

Compensation:

Compensation Range:

$50,000 – $100,000
