Position Description

The Physician provides comprehensive preventive and primary care to BCHC patients in a manner consistent with organizational policies and procedures, including meeting provider productivity and clinical quality outcome standards.
The incumbent is accountable for the clinical leadership of their Patient Centered Medical Home team. The Physician must assess, diagnose, and treat a large variety of acute and chronic conditions and promote wellness within the scope of general practice. The Physician’s health care services must be provided with a Patient Centered Medical Home approach, including working with a panel of patients to optimize their health outcomes and providing care in a cultural and linguistically appropriate manner. The Physician will ensure that practice is HIPAA compliant. The Physician will be an active participant in health center quality and strategic planning activities.
Provides direct preventive and primary care across the lifespan. This includes providing health maintenance, acute care, and chronic disease management services at the health center or in outreach settings in a manner consistent with Patient Centered Medical Home principles.
Provides direct medical care for each patient, including:
Complete medical history
Physical examination
Recommended screenings
Diagnosis(es)
Plan of care including appropriate treatment and/ or referral
Follow-up/ongoing care
On-call availability (shared responsibility with other physicians)
Provides high-quality, culturally-competent, linguistically appropriate care.
Thoroughly and correctly documents medical encounters in health center’s electronic medical record system.
Correctly codes medical encounters according to level of patient encounter and appropriately links codes to the medical diagnoses.
Orders referral resources in diagnosis and treatment (i.e. labs, x-ray) in an appropriate and cost efficient manner and ensures referral tracking procedure is completed.
Refers patients in need of specialty care and ensures referral tracking procedure is completed.
Provides health education and demonstrates effective communication.
Achieves BCHC provider productivity and clinical quality outcome standards as defined in BCHC’s provider practice agreement.
Will share call, one week at a time, equally.

Requirements
Minimum Education Required:
A degree of Doctor of Medicine or Osteopathy or certified by the ECFMG, if an international medical student.
Board eligible or Board certified in a ACGME approved residency.
The candidate is required to maintain continuing medical education credits that satisfy requirements for re-licensure.
Preferred:
Completed an approved residency program in Family Medicine or Internal Medicine/Pediatrics
Spanish language fluency
Previous work in community health centers
License: Must be licensed to practice medicine in the state of Kentucky, or eligible for licensure, as required by KRS 311.560, and must hold a valid DEA certificate. These credentials and their associated fees are solely the responsibility of the physician.
