Community Health & Wellness Center has a full-time opportunity in our new Canaan facility for a BE/BC Family Medicine physician to join a respected and expanding system of skilled providers that deliver a full range of health and specialty care services at convenient locations throughout the northwest corner of Connecticut. • Our Primary Care Network is a NCQA Level 3 recognized Patient Centered Medical Home. • Office locations in Torrington, Winsted and Canaan (opening soon!) • Out-patient community care only; no inpatient responsibilities. • 5-day work week schedule with shared call coverage. • H1B visa sponsorship available. No weekends, 9 paid holidays.
