The Medical Director for the Home-Based Primary Care Program (HBPC) is the senior clinician and is responsible for the overall medical care delivered by the HBPC Team.

The Physician is academically and clinically prepared to perform a broad spectrum of patient care services at the expert level. In addition to providing primary health care, the MD serves as a consultant to nursing and medical staff for patients with interdisciplinary team members in providing quality patient care and education. The MD serves on medical center, VISN and/or national committees and assists in the development, evaluation, and communication of policies and standards to all healthcare disciplines. The incumbent will provide direct patient care which includes assessments, diagnosis, treatment and outcome measures of healthcare problems. This includes the following: initiating and adjusting therapeutic regimens, performing histories and physicals, ordering diagnostic and therapeutic tests/studies, initiating consults and referrals, and prescribing medications/supplies in accordance with VA policy, VA formulary, Federal & state regulations and scope of practice. Executes position responsibilities that demonstrate leadership, experience, and creative approaches to seriously ill veterans in the home setting. Amplify the awareness of the concepts, role and philosophy of Home-Based Primary Care services among ambulatory and acute care clinicians. Facilitate access to appropriate supportive care services. Plan and direct educational experience for medical students, residents, fellows and Nurse Practitioners assigned to Home Based Primary Care. Collaborating with the HBPC Program Director to provide counsel to the Geriatric leadership team regarding process improvement, program development, and policy decisions related to Home Based Primary Care services. Collaborating with other HBPC Medical Directors, VISN leadership, and VA Central Office staff on program development issues. Providing clinical input and oversight for all patient treatment plans. Being readily available to the team members for collaboration when medical or other problems arise. Participating in HBPC team meetings. Keeping the HBPC team apprised of medical care advances and practice standards. Arranging physician coverage and communicating the plan of coverage to the HBPC team.

$250,000 – $320,000 salary

Sign-on bonus may be available for well-qualified candidates

Up to $200,000 assistance for student loans

Contract buy-out opportunities in some situations

50-55 days of annual paid time off per year (26 days of annual leave; 13 days of sick leave; 11 paid Federal holidays; and possible 5 day paid absence for CME)
