PT Dentist Opportunity – Flexible Days!

This Community Health Center seeks a capable and motivated General Dentist who can provide best quality services to the medically underserved population. We provide high-quality, patient-centered, comprehensive primary care in a setting that is welcoming and responsive to the cultural, language and health needs of the communities we serve.

Our network is comprised of multiple medical and dental clinics as well as outreach sites across rural and urban communities in northern and central Illinois.

Our team includes providers who specialize in family medicine, internal medicine, infectious disease, psychiatry, OB/GYN and pediatrics.

This position is 2-3 days per week.

The Dentist will be available to work for 9 hours shift. We can be flexible with day of practice.

Short Term Disability Insurance. Provider shall be entitled to 60% of pre-disability basic monthly salary herein established and according to Short Term disability policy and subjected to all eligibility.

Long Term Disability Insurance. Provider shall be entitled to 60% of basic monthly salary herein established and in accordance with Long Term disability policy and subjected to all eligibility and where applicable.

Employee Assistant Program. Dearborn National for disability resources services, administered by ComPsych corporation. Completely covered with cost to provider.

Profit Sharing Plan. 403B plan through Mutual of America.

CME/Professional Meetings. Provider shall be entitled to attend continuing medical education courses, seminars and professional meetings as may be required to maintain Provider’s license and board certification or to maintain Provider’s technical sufficiency. 5 days and $1,250. License/Dues/Memberships.
