Position Description
Recovery Specialist play a vital role within the BH team, as they provide evidence-based psychotherapy and case management services to patients of the health care center! Primary responsibilities include (but are not limited to):
Conducting comprehensive behavioral health assessments to understand the unique needs and goals of patients.
Collaborating with patients to develop individualized treatment plans which include a life goal and a health goal, to build motivation and progress towards completion of these goals in a a database per Mental health Board – Circle of Care grant requirements.
Deliver evidence-based therapeutic interventions for a variety of behavioral and physical health conditions, including anxiety, depression, chronic pain and substance use.
Work as part of a team and communicate with primary care providers, psychiatrists, and other team members to integrate behavioral health services into overall patient care plans.
Coordinate information flow between the PCP, BH providers, CHW, and other FCHC staff within the Circle of Care program.
Qualifications:
Master’s degree in Social Work, Professional Counseling or Marriage and Family Therapy – REQUIRED.
Eligible for Professional licensure (LCSW, LPC, or LMFT) within the first year of employment of FCHC ~ REQUIRED.
Previous work experience providing behavioral health services – PREFERRED.
Strong desire to work in urban community health setting – PREFERRED.
Strong interpersonal and communication skills – PREFERRED
