Position Description
As a Social Worker, you will provide support to people receiving services at the Life Skills & Transition Center, including those in the Youth Transition Services. Services include psychosocial assessments, individual, group, and family therapy, admission and discharge planning, and consultation. In addition, this role includes supporting the agency outreach mission for discharge planning and admission prevention.
To thrive in this position, you must be proficient in use of a computer, as well as possess good time management and organization skills. In addition, you must be willing to work with a multidisciplinary team, as this position requires coordination between multiple agencies and members of the person-centered team.
To be considered for this position, you must have your licensure as a Licensed Baccalaureate Social Worker (LBSW) by the ND Board of Social Work Examiners; and one year of professional human services work experience as a social worker, human relations counselor, developmental disabilities case manager, mental illness case manager, vocational rehabilitation counselor, activity therapist, addiction counselor, registered nurse, employment counselor, or a similar professional level position in the public or private sector, or a Master’s Degree in social worker and licensure as a Licensed Social Worker by the ND Board of Social Work Examiners.
If no applicants meet the above qualifications, consideration will be given to applicants who have passed their ND Social Work licensure exam and are awaiting their license number. Consideration will also be given to candidates who have their LBSW and less than one year of experience.
Location: ND HHS utilizes a blended workplace model. This role will be working on site at the Life Skills & Transition Center in Grafton, ND.
