Job Summary and Qualifications
Responsible for the implementation and coordination of a program that delivers
standardized care to stroke patients across the continuum of care. Collects
and reviews data to identify trends for benchmarking and performance
improvement. Works collaboratively with Hospital and Medical Staff to lead
stroke performance improvement and certification activities in a manner that
promotes a culture of learning and patient safety.
What you will do:
- Participate in Code Stroke Activations
- Attend multidisciplinary rounds
- Assure care paths are followed
- Assess complex stroke patients
- Consult with hospital staff and providers
- Performs concurrent and retrospective chart review
- Analyzes data to create solutions
- Organizes and stores data
- Collaborates with Hospital and Medical Staff for case and data review
- Facilitates process improvement initiatives within the Stroke Interdisciplinary Team Committee
- Pursues expertise in acute stroke management and a firm understanding of secondary stroke prevention
- Maintains 8 Stroke CEU’s annually
- Serves as content expert for hospital staff, providers, patients, families and the community
- Develops and maintains a plan for education
- Collaborates with local EMS partners for community outreach
- Willing to participate in or lead research efforts
- Provides oversight of Stroke Services in accordance with regulatory requirements
- Performs evaluation of the program
- Responsible for compliance to the regulations of The Joint Commission standards for Advanced Primary Stroke Center
- Knowledge: Understanding of disease specific standards, clinical procedures, equipment, and policy.
- Analysis: Ability to perform computerized analyses and use data management, word processing, and presentation software as required.
- Understanding: Familiar with the healthcare environment and hospital administration.
- Organization: Develops and manages documentation systems ensuring accessibility and appropriate maintenance; prioritizes work and the work of personnel.
- Time Management: Manages multiple projects and meets deadlines.
- Attention to Detail: Able to accurately document and communicate issues related to the stroke program.
- Work Independently: Performs functions of the position with minimal or no direction.
- Independent Judgment: Able to independently determine when issues should be directed up the chain of command.
- Confidentiality: Assures confidentiality of issues and documentation in compliance with state and federal laws as well as medical staff bylaws and policies and procedures.
- Communication (strong written and verbal skills): Communicates with a variety of audiences; able to facilitate open and effective communication between staff, providers and administration.
- Interpersonal: Able to work effectively with multiple personalities and cultures; able to take both positive and negative feedback.
# What qualifications you will need:
* Current RN Licensure.
- Associate’s degree in nursing required. Bachelor’s degree in nursing preferred.
- Minimum of two years working in the neurosciences field.
