Conviva Senior Primary Care, a subsidiary of Humana, Inc. is seeking a primary care physician for our clinic in Callahan, Florida.

Total Compensation Package (base pay + bonus) could exceed $300k, depending on experience and location.

A Primary Care Physician opportunity on our team can offer you:

• Outpatient Schedule – Monday – Friday (8am to 5pm)

Required Qualifications:

• Doctor of Medicine or Osteopathic Medicine
• Board certified or Board eligible in Family Medicine or Internal Medicine (Geriatric Medicine experience, preferred)
• Valid, unrestricted license in FL
• Current, unrestricted DEA license

Work Schedule:

Weekdays (8-5)
